Is a seat reservation required on PKP Intercity trains?

It depends on the train category. Reservations are always mandatory on EIP Pendolino services and on any train that crosses into Poland, only recommended on EIC and TLK trains, and mixed on IC trains, where some routes carry a mandatory reservation marked with an R symbol and others are recommended only.

How much does a PKP Intercity seat reservation cost?

A mandatory EIP Pendolino reservation costs 43 PLN if bought in Poland or around 10 EUR if bought abroad. Reservations for international trains crossing the Polish border typically cost a few euros, for example around 4 EUR to Berlin, Leipzig, or Frankfurt (Oder) and around 3.50 EUR to Munich, Vienna, Prague, Bratislava, or Budapest.

Do Interrail and Eurail passes work on all Polish trains?

Not on the ICN Nieśpieszny heritage service, which does not accept Interrail or Eurail passes at all, and several regional operators such as Koleje Mazowieckie and the Warszawska Kolej Dojazdowa also do not honour these passes. Check acceptance before boarding if a connecting leg of your journey shows one of these regional brands.
